Documentation of Serious Illness

If a student misses more than one week of classes due to an illness and is being monitored by Student Health Services, we will provide the student with written documentation of the time frame of the illness.

Students followed by medical or mental health care providers (off campus) must contact their provider for appropriate medical documentation.  This documentation should be submitted to the Student Ombudsman person in Room 2008, Webb Center at (757) 683-3442.
