Pre-Entrance Health History Requirements

All full-time students entering Old Dominion University must submit a completed Health History Form and provide evidence of having received 2 doses of the Measles/Mumps/Rubella (MMR) vaccine, 1 dose of Meningitis vaccine (or the signed waiver form), 3 doses of Hepatitis B vaccine (or the signed waiver form), and a current Tetanus/diphtheria (Td) or Tetanus/diphtheria/acellular pertussis (Tdap) vaccine booster within the last 10 years.

Students may also be required to show proof of a recent PPD Tuberculosis Skin test or IGRA blood test based on risk factors which are on the Tuberculosis Risk Assessment on the Health History Form. This form must be returned by mail or faxed to Student Health Services by August 1 for full-time students entering in the fall and January 2nd for full-time students entering in the spring.  If this completed information is not received, a hold will block the student's registration for second semester.  Address and fax information is on the form, which can be downloaded below. The form may also be returned after hours to the mail slot located on the right side of the Student Health Services front door.
