Confidentiality

All medical records and communication between patient and staff are confidential. Information cannot be released from Student Health Services without the patient's prior consent with the exception of a subpoena, a court order, or information disseminated in a life threatening emergency. To access the Student Health's Records Release Authorization form, click here.
